This page explains what happens to information you submit through this website.

Information we collect

We collect only the information you choose to send us through the contact and order inquiry forms: typically your name, organization, email address, phone number and the content of your message. Hospital staff taking part in a pilot program may also complete the staff feedback survey described below.

How we use it

We use this information solely to respond to your inquiry and, where relevant, to arrange an order. We do not sell it. Survey responses are used as described in the next section.

Staff Pilot Feedback Survey

Physicians, nurses and other NICU staff at hospitals taking part in a NICU Jargon Rx pilot program can share feedback through our staff survey. The survey is intended for program-improvement feedback. It is not designed to collect protected health information, and it should not be used to send information about any patient or family.

What we collect. When you submit the survey we receive:

• your hospital or institution, unit or department, and professional role;
• your answers to the survey’s rating questions and any written comments;
• your name and email address, only if you choose to provide them;
• the date and time of your submission.

Our website host may also record technical information sent with the submission, such as your IP address and browser details. Because your institution, unit and role are recorded, responses are not anonymous, even when no name or email is given.

Why we collect it. To understand how the cards are used in practice, identify barriers to implementation, and improve NICU Jargon Rx and the way it is provided to NICU teams and families.

How it is processed and stored. Survey responses are received and stored by Netlify, the company that hosts this website, using its form service. Netlify screens submissions for spam using an automated filtering service. Each submission also generates an email notification containing the responses, sent to the NICU Jargon Rx business inbox. Responses may be exported to a spreadsheet for analysis.

Who can see it. Access to survey responses is restricted to NICU Jargon Rx personnel we have authorized.

How it is analyzed. Responses may be grouped and analyzed by institution, by professional role, and in aggregate across participating institutions, to improve the educational resource and support future hospital implementations.

Please do not include patient information. Do not enter patient names, medical record numbers, dates of birth or any other information that could identify a patient or family. Participating institutions should follow their own policies when collecting, storing, analyzing or using feedback, particularly if activities may constitute research.

Please do not send health information

This website is not a secure channel for personal health information. Please do not submit details about an individual baby, patient or medical situation through these forms. For questions about a child’s care, speak with their healthcare team.

Third-party services

• Form submissions are delivered by a third-party form-processing service.
• Staff survey responses are received and stored by Netlify, our website host.
• Typefaces are served by Google Fonts.
• Video, once published, may be served by a third-party video platform.
